Polaroid Macro 5 SLR - 1200 Camera Instant Film 5-in-1 Close Up System. Allows you to take 5 different ranges of pictures. There's a setting for 0.2x, 0.4x, 1x, 2x and 3x.

Designed for medical documentations and other things requiring close up photographs instantly. I did not have film to test it, I put fresh batteries in and it did not turn on. It may be a simple repair, I'm not sure, I just do not have time to look into it. There is a crack in the plastic by one of the flashes as shown in the picture. Really cool unique camera if it's working.
